It is important that the local union and/or employee follow up on the request for a waiver by requesting a copy of the completed form to include the backside of the form.
On the backside of the form, it requires additional fact circumstances from the Postmaster/Installation Head.
In most cases, it supports the employee/former employee’s reasoning for why the overpayment should be waived.
This completed form should be in your Article 15, 19 and 28 grievance.
You need to also make sure that you later include the final disposition of the waiver request including the appeal in your grievance file from USPS Gary Laurant.
